Terms & Conditions

1. STUDENT BEHAVIOR AND CONSEQUENCES

The parent/guardian understands that if my child collects three (3) warnings (e.g for behavior or non-participation) they may be removed from the program.

First Negative Action: Verbal warning given by STARBASE Guam staff.

​

Second Negative Action: The student is removed from the activity and the student will discuss actions with the STARBASE Guam Director. Parents will be contacted. 

​

Third Negative Action: The student is removed from the STARBASE Guam Summer Camp. 
 

  • No gum is allowed at STARBASE Guam

  • STARBASE Guam students are not authorized to leave the STARBASE Guam facility for any reason without a Teacher Chaperone or STARBASE Guam staff member.

  • For any negative action which is severe or creates a risk to other students, the STARBASE Guam staff maintains the right to immediately remove a student from the activity and not invite the student back to STARBASE Guam without proceeding through the Code of Conduct.

 

The parent/guardian understands and acknowledges that their child, identified in the registration, understands and agrees to protect all property (device/equipment), at all times.  All classroom property must be used for its identified purpose and must be returned to the STARBASE instructor at the end of the class session.

 

The parent/guardian understands and acknowledges that all classroom devices/equipment are STARBASE Academy property, and is provided as an educational tool/resource.

2. HOLD HARMLESS AGREEMENT

Please Note: The statement following is legal and binding. Do not alter or modify in any way.

 

In the event of an accident, illness or injury and the persons on the first page cannot be reached. I hereby give DoD STARBASE personnel permission to take action as deemed necessary in the best interest of my child. 

 

Furthermore, I take full responsibility for any damage that might occur to DoD STARBASE property caused by my child. I understand this program is designed for “hands-on” activities, teamwork, self-confidence, and visits to military work areas. I agree not to hold DoD STARBASE Guam, the Department Military Affairs, and/or its staff or representatives liable in any way for mishaps which could occur due to the nature of the activity in which my child is engaged. I also understand that the DoD STARBASE staff reserves the right to terminate the participation of my child when it is deemed to be in the best interest of either the child, or the DoD STARBASE Academy, as determined by the DoD STARBASE staff.

3. FINANCIAL OBLIGATION

The parent/guardian may be liable for the cost of repair or replacement of the STARBASE Guam Academy property (device/equipment) should it be damaged due to student negligence or intentional damage. Parent/Guardian will be notified immediately by the STARBASE Guam Academy Director and/or school administrator/teacher. The parent/guardian has up to ten (10) business days from the day of notification to address and resolve the cost of repair or replacement.

DoD STARBASE Guam admits students of any race, color, nationality, and ethnic origin to all the rights, privileges, programs, and activities generally accorded or made available to students at DoD STARBASE Guam. It does not discriminate on the basis of race, color, nationality, and ethnic origin in the administration of its educational policies and admissions policies.

PUBLIC AFFAIRS RELEASEThe sponsor of the identified child, hereby give DoD STARBASE, its sponsoring agencies, and those acting under their permission and on their authority, the absolute right and permission to copyright and/or use and/or publish photographic portraits, pictures, or videos of the individuals in which my child may be included in whole or in part, or composite or distorted in character or form, in conjunction with the child's own or fictitious name, or reproductions thereof in color or otherwise, made through any media for art, advertising, trade, or any other lawful purpose whatsoever. Parent/guardian hereby waive any monetary or other rights that I or my child may have to inspect and/or approve the finished product of the advertising copy that may be used in connection with the use to which it may be applied.

Parent/Guardian releases, discharges, and agrees to save harmless DoD STARBASE, its sponsoring agencies, and those acting under their permission and upon their authority, from any liability by virtue of any building, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said picture of in any processing tending toward the completion of the finished product.

 

Parent/Guardian hereby consents to the release of said portraits, pictures, videos, or motion pictures to other broadcast media, such as non-governmental television, cable, and/or radio stations.

 

Parent/Guardian further assigns to the said organization all rights and titles and interests in the above-described video recordings, motion pictures, photographs for any further use in the area of motion pictures, video recordings, publicity pictures, etc.

Parent/Guardian understands and agrees that said organization may maintain video recordings, photographs, etc. for training purposes and archives.

​

Parent/Guardian has read the above authorization and release prior to its execution and am fully aware of its contents.
